The SPA dialplans execute on the first match so if you put the 1900
block ahead of the long-distance allow, you should be good.  In general,
place more specific rules (911S0) ahead of less (xxxxxxxx.).  You can
test this by changing your 1900 block to a 1905 block and try to call
someone in Hamilton.

I find it more secure (and easier to manage) to have a dialstring
without the wildcard xxxxxxx.  That way only what you have explicitly
defined is dis/allowed.  Add in a rule for international calling (if
desired) and that ought to cover all your bases. 

When possible, I like to deal with dialplans in Asterisk/FreePBX.  It is
easier and then all the dialplan info for your system is centrally
stored.  It doesn't sound like this is an option for you here.
